FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION NOTICE OF AGENCY ACTION

The Florida Department of Environmental Protection (“Department”) gives notice of issuance of a combined environmental resource permit (“Permit”), confirming regulatory, proprietary, and federal authorization, to Scott Mandel at 2732 Southeast County Road 21-B, Melrose, Florida 32666, Section 35, Township 8 South, Range 22 East, Bradford County, for the construction of a 164’ long single family dock, with two boat slips and a second story observation platform, preempting 1979.5 sq. ft. of sovereign submerged land within Lake Santa Fe, Alachua County, Florida. The Department’s File Number for this authorization is 04-0290349-003-EG. This action is final and effective on the date filed with the Clerk of the Department unless a petition for an administrative hearing is timely filed under Sections 120.569 and 120.57, F.S., before the deadline for filing a petition. The Town of LaCrosse will be accepting bids for the construction of a new parking area located at the Town of LaCrosse Fire Department. Scope of work for new parking area: (please see the attached diagram for reference) 1. New Parking area On the West entry wall of the fire station- demo the existing broken concrete sidewalk in an approximate 24’ deep by 31’ wide area and haul away. Remove existing flagpole and save for reuse- provide labor and materials to reinstall in a location specified by the Fire Department, on site. Excavate to 8” beneath existing asphalt- and compact existing subgrade to 98% compaction in preparation for concrete. Pour 8” of 3,000 psi concrete with vapor fiber mesh and/ or 5x5 welded wire mesh. Ensure slopes and side slopes meet code requirements. Stripe per code for two new parking spaces including one ADA space with walkway, and one standard parking space. Install 3 curb stops for 3 new parking spaces. 2. Parking area trip hazard/ swale repair Install one approximately 21’ long 6” diameter PVC sewer pipe in the bottom of the existing concrete swale on the Southeast corner of the Fire Station from the South east corner-East. Tie in the 6” drain pipe into existing storm structure. Pour an approximately 4’ wide x 20’ long area over the new PVC sewer pipe with Fiber mesh AND 5x5 Welded Wire Mesh reinforced 3,000 psi concrete within the existing concrete swale. 3. Parking separation fence to prevent further damage Remove approximately 47’ of chain link fence and save for reinstallation. Re-install 38’ +- of existing fencing (salvaged for reuse) on the East side projecting from the current Fire Department fence southeast corner in a south direction following the current plane of the fire department fence. Provide and Install approximately 35’ of 9 gage chain-link fence 6’ tall between the existing park fence to the south, in a north direction to connect to the existing corner post on the south east corner of the Fire Department side. New fencing should contain a continuous top rail, and tensioning wire at the bottom and will replace the cable gate currently in place in the west end location. Bidders will meet for a pre-bid tour at the west side of the LaCrosse Fire Station, 20421 N SR 121, on Wednesday, March 9th, at 1pm. FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION NOTICE OF AGENCY ACTION

The Florida Department of Environmental Protection (“Department”) gives notice of issuance of a combined environmental resource permit (“Permit”), confirming regulatory, proprietary, and federal authorization, to Scott Mandel at 2732 Southeast County Road 21-B, Melrose, Florida 32666, Section 35, Township 8 South, Range 22 East, Bradford County, for the construction of a 164’ long single family dock, with two boat slips and a second story observation platform, preempting 1979.5 sq. ft. of sovereign submerged land within Lake Santa Fe, Alachua County, Florida. The Department’s File Number for this authorization is 04-0290349-003-EG. This action is final and effective on the date filed with the Clerk of the Department unless a petition for an administrative hearing is timely filed under Sections 120.569 and 120.57, F.S., before the deadline for filing a petition. On the filing of a timely and sufficient petition, this action will not be final and effective until further order of the Department. The Town of LaCrosse will be accepting bids for the construction of a new parking area located at the Town of LaCrosse Fire Department. Scope of work for new parking area: (please see the attached diagram for reference) 1. New Parking area On the West entry wall of the fire station- demo the existing broken concrete sidewalk in an approximate 24’ deep by 31’ wide area and haul away. Remove existing flagpole and save for reuse- provide labor and materials to reinstall in a location specified by the Fire Department, on site. Excavate to 8” beneath existing asphalt- and compact existing subgrade to 98% compaction in preparation for concrete. Pour 8” of 3,000 psi concrete with vapor fiber mesh and/ or 5x5 welded wire mesh. Ensure slopes and side slopes meet code requirements. Stripe per code for two new parking spaces including one ADA space with walkway, and one standard parking space. Install 3 curb stops for 3 new parking spaces. 2. Parking area trip hazard/ swale repair Install one approximately 21’ long 6” diameter PVC sewer pipe in the bottom of the existing concrete swale on the Southeast corner of the Fire Station from the South east corner-East. Tie in the 6” drain pipe into existing storm structure. Pour an approximately 4’ wide x 20’ long area over the new PVC sewer pipe with Fiber mesh AND 5x5 Welded Wire Mesh reinforced 3,000 psi concrete within the existing concrete swale. 3. Parking separation fence to prevent further damage Remove approximately 47’ of chain link fence and save for reinstallation. Re-install 38’ +- of existing fencing (salvaged for reuse) on the East side projecting from the current Fire Department fence southeast corner in a south direction following the current plane of the fire department fence. Provide and Install approximately 35’ of 9 gage chain-link fence 6’ tall between the existing park fence to the south, in a north direction to connect to the existing corner post on the south east corner of the Fire Department side. New fencing should contain a continuous top rail, and tensioning wire at the bottom and will replace the cable gate currently in place in the west end location.
